We have had a few like that! The most common is the tear off cap from an additive bottle that gets poured into the tank and the "tear off" falls into the tank. This then gets sucked up by the stand pipe. So be careful what you offer to the filler neck!
The other is the shirt button that just happens to fall off the cuff into the tank. Often the same diameter as the stand pipe it gets sucked up the tube to restrict the fuel flow. Once the truck falters to the side of the road the button falls out and floats into the bottom---Only to be sucked up again at a later date to confound the owner/driver AND the machanic trying to find the fault.
Lovely these faults.
